My Gym Fun Night in Support of "Yes For Scituate Middle School"!
Use this form to sign-up for this important fundraising event! Your kids will have a blast engaging in gym activities with our staff and a pizza dinner will be provided. Payment by check will be made at the door. We can take 25 children, so we hope to raise $500. If, on the day of the event, your child(ren) is unable to attend, please still consider donating so that we can meet this $500 goal.

POLICIES:
- The cost (donation) per child is $20.
- Payments will be accepted when you arrive.
- Payments must be made by check and written to "Yes for Schools and Safety".
- Children must be between 3.0 to 8.0 years old to attend.
- Parents may stay or drop-off.
