How Do I Change My Mother’s Name on my PSA Birth Certificate?

Sofie has always known that her mother’s name is Emelita.  That is the name written on all her mother’s IDs and on the birth certificates of Sofie and her siblings.  However, when Aling Emelita requested for a copy of her PSA birth certificate, they were shocked to see that her name is actually not Emelita but Maria Rosario!  All other information on her birth certificate is correct, her birth date, birthplace, and the names of her parents.  Everything, except her name.

They discovered this in the early ‘90s when Aling Emelita was preparing to work abroad.  Since she was in a hurry to get a passport, she resolved to use her Maria Rosario birth certificate and was granted a passport as Maria Rosario.

When Sofie graduated from high school with flying colors, her parents gifted her and her siblings a trip to Tokyo Disneyland.  Sofie and her three siblings, all minors, trooped to the DFA to get their passports.  But they were all horribly disappointed when their applications were put on hold because of the inconsistency in their mother’s name as written on their birth certificates and the name that appears on their mother’s birth certificate.  On their birth certificates, their mother is Emelita.  But on Aling Emelita’s, her name is Maria Rosario.

How does one correct their parent’s name as it appears on their birth certificate?

While it is clear that Aling Emelita has chosen to simply adopt her name as written on her birth certificate (Maria Rosario), she failed to work on correcting the details on her children’s birth certificates, where her name is still written as Emelita. 

Sofie and her siblings need to have their birth certificates corrected to reflect their mother’s name as Maria Rosario.  Here’s what they need to do:

  1. They need to go to the Local Civil Registry of their birthplace and request for correction of their mother’s name as it appears on their birth certificates.
  2. They must bring a copy of Aling Emelita’s PSA birth certificate where her name is written as Maria Rosario.  Any other government-issued ID of Aling Emelita (as Maria Rosario) will likewise help in further solidifying their petition.
  3. Fees vary according to municipality and payments must be made only with the city or municipal hall’s cashier.  All payments must be issued with a government receipt.  Be wary of fixers.
  4. When the petition is successfully filed by the LCR, Sofie and her siblings will be given a copy of the Finality and Endorsement pertaining to the correction they requested.
  5. After three to six months, they can begin following up with the PSA for the corrected copies of their birth certificates.  When making a follow-up, bring the Finality and Endorsement documents from the LCR.
  6. The first corrected copies of their birth certificates must be claimed at the PSA head office in Sta. Mesa, building 2.  All other succeeding copies may be requested online at www.psahelpline.ph.

    1. Paanong ibang pangalan ang nilagay sa birth certificate mo? Bakit pumayag ang mother mo na hindi niya pangalan ang nakalagay sa birth certificate mo?

      Anyway… Nasa sa iyo yan kung kaninong “Mother’s Name” ang gusto mong gamitin.

      Kung ire-retain mo ang pangalan ng stepmother mo sa birth certificate mo, kailangan mong papalitan ang mother’s name sa lahat ng documents mo. Mas madali yun. Pero ang importante, mailagay mo ang pangalan ng tunay mong nanay sa birth certificate mo. Ang tanong: Paano mo mapapatunayan na siya ang tunay mong nanay? 🙂 Subukan mo munang mag inquire sa LCR, tapos sa abogado (kung meron kang kakilala o kaya sa PAO).

My mother’s name real name is Maxima Maria (ito po yung nakalagay sa birth certificate niya) but in my birth certificate (her child), “Emma” po yung nakalagay which is her nickname… I consulted an attorney and sa ganitong case raw po is gagastos ng malaki like hundred thousands which is hindi ko po afford as of now. What should I do sa pag fill out ng mother’s name sa passport ko? Sundin ko nalang po ba yung Emma?

    1. Hi! Not sure with regards the passport (better ask DFA). But my best guess would be to follow anong naka sulat sa iyong birth cert. As to changing your mother’s name sa birth cert mo, go to you Local Civil Registrar’s Office and file for a petition to have it changed and it only cost me Php 1,800.00. Ang lawyer fee lng nagastos ko is 50.00 pesos for notarization. Ang final result of you new Birth Cert is may annotation na ito sa gilid na naka remarks na ang name ng mother mo is to read “Maxima Maria” instead of “Emma”.

      In short, your Birth Cert won’t be totally changed, may remarks lng siya, and it will cost you less than 2K pesos lng po. Ang Local Civil Registrar’s Office na din po ninyo ang mag foforward ng annotated Birth Cert ninyo to the PSA.

    2. Sundin mo na muna kung ano ang nasa birth certificate mo para sa passport application because the DFA will always refer to your birth certificate to verify your personal details.

      Also, try inquiring at the LCR (city hall) para sa correction ng pangalan ng mom mo sa birth certificate. If tama naman ang name ng mother mo sa birth certificate niya, and kung tama din ang name niya na nakalagay sa birth certificate ng mga kapatid mo and marriage certificate nila ng father mo, then that should be enough basis to simply correct her name on your birth certificate. I don’t think it will cost you hundreds of thousands of pesos to get that corrected.

  5. Yung birth certificate po ng mother ko, yung mother’s name nya doon is yung lola nila. Then sa Birth certificate nman po ng kapatid ng mother ko is yung real name ng mother nila. What should we do po? Need lng bka mag ka problem sa proof of relationship.

Name of my mother in my PSA Birth Certificate is Ellen, but per her PSA Cert, her name is Maria which she uses already in all of her documents since obtaining her passport following her birth cert. I have yet to process correction of my birth certificate with LCR to change my mother’s name to Maria.

    Now I’m confused with regards my temporary visa application for Canada, which I will submit this month. Could you advise which mother’s name should I put in my Visa application?

Subukan niyo pong mag inquire sa LCR office kung saan naka rehistro ang birth certificate ng anak niyo para malaman kung pwedeng petition for correction of clerical error lang ang gawin para ma-correct ang pangalan niyo sa birth certificate niya. Dalhin niyo po ang birth certificate niyo bilang patunay ng tamang spelling ng inyong pangalan. Mabuti din na ipa-correct niyo na ang pangalan niyo sa mga ID niyo para consistent ito sa birth certificate ninyo.

Kung isang letra lang ang mali sa first name mo sa iyong birth certificate, pwede kang mag file ng Petition for Correction of Clerical Error sa munisipyo kung saan naka rehistro ang birth certificate. Dalhin mo ang birth certificate copy na may error, ipakita mo sa LCR, at sila ang mag determine kung ano ang requirements at fees.

    Paano po kaya to ? Ngayon ko lang nalaman n ang record ko sa psa ay baby girl ang pangalan ko ..? Pero since birth ay angelica ang gngmit ko n pangalan maging sa mga ids ko .. may anak ako ngaun pwede ko b papalitan ung pangalan ng magulang n nklgay sa birth certificate nya gagawin ko baby girl imbis na angelica ?? Ung mothers name kc n ginamit ng mama nya ay ung name nya sa tita nya n nagpalaki s kaniya pro hnd nkarehistro s psa.un dn gamit nya nun ngaral dw sya.       Pwede ninyong ipa correct yung pangalan ng mother as it appears in the child’s birth certificate. May kopya ba kayo ng PSA birth certificate ng mother niya?

      Kung meron, dalhin niyo ito sa munisipyo kung saan naka rehistro ang anak ninyo at mag request kayo ng correction ng mother’s name. Mag dala din kayo ng iba pang valid IDs ng mother.

      Pag naka file na yung correction sa LCR, bibigyan nila kayo ng Finality and Endorsement. After 3 to 6 months, pwede na kayong mag follow up sa PSA office sa East Avenue QC. Dalhin niyo ang Finality and Endorsement kapag nagf-follow up.

    Ask ko lang po kasi yung sa una kong birth cert, na misspelled and name ng mother ko pero naka pag file na siya for correction at ok na ngayon, then nag apply ako psa birth certificate at nilagay ko din yung corrected name ng mother ko. Now the question is, yung sa psa ko po na inapply, masusunod na po ba yung corrected name ng mother ko?

      Pag nakakuha ka ng bagong copy ng birth certificate, dapat may annotation na to show the corrected name of your mother — PERO — hindi na mapapalitan yung naka sulat mismo sa birth certificate mo. Magkakaroon na lang ng note sa taas or sa side ng document na nag undergo ng correction ang birth certificate mo at ilalagay dun ang correct na name ng mother mo.

    Ang nasa birth cert ko po kasi na name ng tatay ko Joji pero ang nasa birth cert nya ay Jugie.

    Ano po bang petition ang dapat na i-file namin ng mother ko, Petition for Correction of Name or Petition for Change of First Name?

    YOLANDA ang gamit niyang name simula nung nag-aral siya hanggang ngayon at yun din ang nakalagay sa BAPTISMAL niya. Nung kukuha na kami ng birth certificate niya s LCR, no record found daw. Tapos nung nag apply na kami for late registration sabi naman may record na lumalabas kapag inencode na yung name ng mother niya. Ang problema, AMELIA ang nakarehistro sa BIRTH CERTIFICATE tapos mali rin yung first name ng father niya. Tama yung name ng mother nya and yung birth date.

      Kung totally different name ang nakalagay sa birth certificate versus sa actual na ginagamit na pangalan, dalawa ang pwedeng gawin ng petitioner:

      1. Pwede siyang mag issue ng affidavit stating that Yolanda and Amelia are the same persons (Affidavit of Two Disinterested Persons). Pero moving forward, kailangan gamitin na niya yung pangalan na Amelia.

      2. Pwede din siyang mag file ng Petition for Change of First Name para mapalitan na mismo ang nakasulat sa birth certificate niya. At Yolanda na ang gagamitin niya.

      It would be best for you to consult the LCR of the city or town where your child was born (or registered). They can give you the best advise.

      Meanwhile, here are the steps in correcting the parents’ information in a child’s birth certificate.

      Kahit na anong discrepancy sa ating public documents, tulad ng birth certificate, ay maaaring ma-consider as ‘big deal’.

      Pwede niyo itong ipa-correct sa munisipyo; kailangan lang mag pakita kayo ng birth certificate ng father mo as reference for the correct spelling of his middle name in your birth certificate.

Marie nakalagay.. pero ang ginagamit po niya na pangalan at Elisa may LCR at NSO po siya na Elisa.. pero yung sa PSA sabi na blocked na daw po yung Elisa since ang unang narehisto daw talaga at Marie, tapos pinapalitan daw po kasi ng lola ko yung name nya dati dahil ayaw nya nun. Magandang buhay po..government employee po ako. kukuha napo sana ako ng passport ng makita po sa birth certificate ko na Male ang gender ko..nagtanong ako sa local civil registry namin…sabi aabutin daw ng almost 10k ang pagsasaayos nun…tama po ba na ganun ang aabutin na gastos para lang mapalitan ung gender? salamat po sa sagot. 🙂

      Ang fees for correction ay iba-iba, base sa mga munisipyo. Pero hindi naman aabot ng 10k. May mga hihingin sa iyo na documents like medical records, old school records. Ang lahat ng ito ay may bayad, depende sa kung saan mo kukunin. Pero overall, hindi naman dapat umabot ng 10k.

      Para makasiguro ka, mag inquire ka sa munisipyo ninyo, o sa abogado kung may kakilala ka. Kung magbabayad ka, siguraduhing sa kahera ng government agency ka mag babayad, o sa kahera ng munisipyo, at siguruhing mabibigyan ka ng valid receipt para makasiguro kang legitimate ang lahat ng charges nila sa iyo.

      Huwag na huwag kang makiki transact sa mga fixer.

  24. Paano po ung Maria at Ma. ? Dati po ang nakasanayan kong gamitin ay Ma. sa documents ko, sa school records, sa mga IDs. at birth certificate ng anak ko. Pati nung kumuha ako ng SSS no. ay Ma. lng din ang ginamit ko pero hindi nila ako nainform na pinalitan pala nila ng Maria dahil binase nila dun sa birth certificate ko. Nalaman ko nlng nung kukuha ng SSS ID dahil hindi daw pwede ung ID na pinakita dahil Ma. lang nka lagay dito, Kailangan ko daw mag produce ng ID na Maria ang nakalagay, kaya lahat ng documents ko ay inupdate ko at pinalitan ko nlng ng Maria. Ang hindi ko lng po napalitan ay ung sa birth certificate ng anak ko. Magkakaroon po ba ito ng problema kalaunan? Umaasa po ako sa inyong tugon. Maraming salamat po.

      Ang lahat ng annotations sa birth certificate ay mangyayari lamang kapag nakapag file ng petition or any kind of request para ma-correct o may mabago sa iyong birth certificate. Gagawin ito sa munisipyo kung saan naka rehistro ang birth mo.
